This policy explains how your personal information, including sensitive information such as your health information, is collected and used by our business or through our website. We comply with the Australian Privacy Principles contained in The Privacy Act 1988 (Cth) and General Data Protection Regulations (GDPR) to ensure that your information is protected. Please read this policy carefully. When providing your personal information to us you consent to us collecting, holding and using your personal information in accordance to this policy.

Information

Personal information refers to any information capable of identifying an individual, for example name, contact details such as phone number and email address, occupation and health information. We will collect your Personal Information directly from you. However, in some circumstances third parties may provided us with information. For example, personal information may also be collected from other sources, such as:

1. your guardian or responsible person (if under 18); and
2. other involved healthcare providers, such as allied health professionals, hospitals, specialists and pathology services.

We will take all reasonable steps to ensure you are made aware when third parties provide information to us. Additionally, we will not provide your information to third parties without your expressed consent.

If you do not provide us with your personal information when requested, we may not be able to provide our products and services to you.

Purpose of Collecting Information

The primary purpose of collecting your personal information is to maximise the services provided by our business. We may collect, hold, use and disclose personal information for the following purposes:

  • to provide you with our products and services, and when necessary provide integrative healthcare ie working with other health care professionals;
  • to contact and communicate with you;
  • for record keeping and administrative purposes;
  • for analytics, market research and business development, including our Site and associated applications and social media platforms;
  • for advertising and marketing purposes, including promotional information about our products and services and general information you may find of interest; and
  • to comply with our legal obligations and resolve any disputes that may arise.

Sensitive Information
Sensitive information is defined in the Privacy Act to include information or opinion about such things as an individual’s racial or ethnic origin, political opinions, membership of a political association, religious or philosophical beliefs, membership of a trade union or other professional body, criminal record or health information.
Sensitive information will be used by us only:
  • For the primary purpose for which it was obtained
  • For a secondary purpose that is directly related to the primary purpose
  • With your consent; or where required or authorised by law.

Your Personal Information will only be disclosed to persons or organisations who need to know it as part of providing our services to you, where you consent to the use or disclosure or where we are required by law to make a disclosure.

Security of Personal Information
Your Personal Information is stored in a manner that reasonably protects it from misuse and loss and from unauthorised access, modification or disclosure.
When your Personal Information is no longer needed for the purpose for which it was obtained, we will take reasonable steps to destroy or permanently de-identify your Personal Information. However, most of the Personal Information is or will be stored in client files which will be kept by us for a minimum of 7 years.

Should you require, you may access the Personal Information we hold about you and to update and/or correct it, subject to certain exceptions. If you wish to access your Personal Information, please contact us in writing.
In order to protect your Personal Information we may require identification from you before releasing the requested information.
